The family of a man killed by his cousin - who was suffering with severe mental ill-health - want answers over the axe attacker's care prior to the tragedy. Ryan Lowry's loved ones there were 'collective failings' and a 'complete lack of joined-up thinking between agencies' in the 'care and supervision' of Ashley Rowen.
Mr Lowry, 36, died after Rowen, then 31, chased him down the street and struck him 13 times with an axe on Moss Lane in Partington, Trafford in February, 2020.
Tree surgeon Rowen, also known as Ashley Glennon, had been diagnosed with paranoid schizophrenia, pleaded guilty to manslaughter on account of diminished responsibility and was sentenced to an indefinite hospital order in January 2021.

An inquest will be held into the death of Mr Lowry, from Marple, Stockport, in March next year and will be heard before a jury. At a pre-inquest review hearing on Wednesday (October 13), assistant coroner Adrian Farrow heard arguments over whether that should be what is known legally as an Article 2 inquest.
These are hearings where Article 2 of the European Convention on Human Rights, the right to life, is invoked and are enhanced inquests that look into the wider circumstances of somebody's death.
The court heard the family say 'there wasn't joined up thinking, there wasn't communication between agencies' and that there was 'conduct, or rather a pattern of behaviour [from Rowen] that didn't trigger a response.'
Mr Lowry's widow Michelle and brother Daniel were present for the hearing at Stockport Coroners' Court.
