A Scots health board has been fined £220,000 over the deaths of two vulnerable patients who died after falling from hospital windows.

An 11-year-old boy has died after being hit by a bin lorry in Edinburgh. The youngster was riding a bicycle when he was struck on Whitehouse Road, near the junction with Braehead Road, at around 8.20am on Friday, 1 March.
Tragically the Cramond Primary School pupil was pronounced dead at the scene. The driver of the bin lorry was uninjured.
One eyewitness told the Daily Record: "It happened during the school run this morning. We saw the buses backed up first and then loads of police cars and ambulances.
"It has really shaken the community, especially happening so close to the school with so many kids around."
Another eyewitness said: "Around 8am we could see something had happened outside but were not sure what. A lot of traffic was outside and some parents with kids going to school.
"It became clear something serious had happened. You would never expect something like this to happen, especially so close to a school. It’s awful what has happened and everyone is thinking about the boy's family."
Helen Donaldson, Cramond Primary School head teacher, said: “We are utterly devastated by the tragic news of the sudden death of one our pupils. Our thoughts are with his family and friends during this incredibly difficult time. He was always smiling. He was a positive, friendly and curious member of our school community. He was kind and always eager to support others, and will be greatly missed by all of us.”
Inspector Andrew Thomson, of Police Scotland’s Road Policing Unit, said: "Our thoughts are very much with the boy’s family and friends at this time. Our enquiries are ongoing to establish the full circumstances and I would urge anyone who may have information which has not yet been passed to Police to get in touch.
