The family of a teenager found dead near the hospital she was receiving treatment has told an inquest of 'systemic failures' in the days leading up to her death.

Residents in Bolton have spoken of their shock after a body was found yards away from a school. Police were called to a wooded area off Chorley New Road in Horwich following the grim discovery shortly before 7.30am this morning (Friday, July 21).
The piece of grassland, which has a path that runs from Brazley Community Centre to Chorley New Road, faces the entrance to St Joseph's RC High School.
The area was taped off and parents and pupils were greeted by a number of emergency service vehicles, including a forensic crime scene investigation van, as they arrived at the school for end-of-year assembles. A private ambulance later attended and the man's body was taken away.

He has not yet been identified, Greater Manchester Police (GMP). said A spokesman for the force said the death was not being treated as suspicious.
The school moved to reassure parents telling them that all students were safe and there was no reason for them to be alarmed.
"This incident is an isolated incident and has nothing to do with school, staff or pupils, but police vehicles are parked next to school whilst dealing with it. Thank you," a message on the school's social media pages read.
People living nearby expressed their shock and sadness at what had happened. One man said there was an ambulance, another paramedic vehicle and four police vehicles.
"We knew then something had obviously gone amiss," he said. "But to hear someone has died is very shocking. When anyone finds a body it's shocking and of course very sad" he added.
