A mum died just months after doctors failed to spot the signs of a brain tumour.
Katie Flynn was just 34 when she died after finally being diagnosed with cancer, reports the Echo. A doctor at the Walton Centre in Liverpool initially failed to spot the tumour until it had grown by 10mm in size.
On Wednesday, an inquest held at Gerard Marjella Courthouse in the Merseyside city heard there were a number of "missed opportunities" and failures in relation to Katie's care, including post-surgery.
Area Coroner Ms Anita Bhardwa concluded death by misadventure, contributed to by neglect, adding that a Regulations 28 report - also known as a prevention of future deaths report - would be sent to the Society of British Neurological Surgeons and the NHS improvement department.
Ms Bhardwa said: "It’s an absolute tragic set of circumstances that’s left two young children without their mum."
Katie, from Bootle in Liverpool, was experiencing pain - including numbness and weakness in her arms and legs. That is when she was found to have a syrinx - a fluid filled cyst that forms within the spinal cord - on November 30, 2020.
She was referred to a specialist spinal clinic followed by a pain team - but when she continued to experience a variety of symptoms including pins and needles in her arms and legs, Katie was sent for an urgent MRI scan on August 31, 2021. The inquest heard a doctor at the Walton Centre failed to spot the signs of the tumour on the scan.
