The Justice Department’s watchdog said Tuesday (June 27) that a “combination of negligence and misconduct” allowed the disgraced sex offender to take his own life at a federal jail in New York City while awaiting his trial on sex trafficking charges, via AP.

Inspector General Michael Horowitz pointed to the federal Bureau of Prisons’ failure to assign him a cellmate after his previous one left, as well as problems with surveillance cameras, as factors in his death in August of 2019.
The Inspector General added that Jeffrey was left in his cell with too many bed linens, a security issue, which were used in his suicide. As with previous investigations, he reported that there was no indication of foul play, despite conspiracy theories about his death.
He also identified 13 employees with poor performance and recommended charges against six workers. Only two that were tasked with guarding Jeffrey Epstein were charged, avoiding jail time in a plea deal after admitting to falsifying logs.
