A man who was recalled to prison was found dead in his cell at HMP Forest Bank after just nine days of freedom. Jonathan Pisani, who was 46, was convicted of firearm offences in August, 2021, and sentenced to 15 months in prison.
He was released on January 4, 2022, but recalled to prison just nine days later, on January 13, for breaching his licence conditions and sent to Forest Bank in Pendlebury, Salford.
A published report by the Prisons and Probation Ombudsman (PPO), which investigates deaths in custody, said Mr Pisani, who had a history of illicit drug and alcohol use, died six days after arriving at the prison - on January 19, 2022.

The report said his cell mate fell asleep at around 8pm the night before after he had smoked 'spice', described as a type of psychoactive substance. But he said Mr Pisani did not smoke any illicit substances that night or at any other time they had been sharing a cell.
Mr Pisani was found 'unresponsive in his cell' at 7.05am the following morning and later pronounced dead by a GP. A post-mortem examination gave Mr Pisani's cause of death as drug toxicity, the report said. A coroner gave a narrative conclusion at an inquest into his death held earlier this month.
The report said: "Mr Pisani had been at Forest Bank since January 13, during which time he had been receiving both alcohol and drug detoxification.
"He had a history of illicit drug and alcohol use, but no concerns were raised about him using illicit substances during the short period he was in prison. He was said to have been compliant with his detox regime. HM Chief Inspector of Prisons and the Independent Monitoring Board both
