A number of 'gross failings' by the Greater Manchester Mental Health NHS Foundation Trust (GMMH) contributed to the suicide of a former rugby player, an inquest has found.
Daniel O'Neill was found unresponsive at his home in Weaste Lane, in the Seedley area of Salford, on November 29 last year, Bolton Coroner's Court heard.
