K.J. Yossman Frank Kozik, the iconic graphic artist behind album covers including The Offspring’s “Americana” and Queens of the Stone Age’s self-titled debut, has died. He was 61 years old. Kozik’s death was confirmed by his wife Sharon, who did not give a cause of death but said his passing was “unexpected.” In a statement she said: “We are devastated to inform you that Frank Kozik passed away unexpectedly this past Saturday. Frank was a man larger than himself, an icon in each of the genres he worked in. He dramatically changed every industry he was a part of. He was a creative force of nature. We are so beyond lucky and honored to have been part of his journey and he will be missed beyond what words could ever express.”

As a teenager, he moved to the U.S., living first in Sacramento with his father before joining the Air Force, which saw him stationed in Austin. A self-taught artist, in his late teens he began creating artwork for friends’ bands, starting with black and white flyers pasted to telephone poles. He soon moved into concert posters, including large silkscreen prints, for bands such as Pearl Jam, The White Stripes, The Beastie Boys, Green Day, Neil Young and Nirvana. Longtime Grammys executive producer Ken Ehrlich worked with Turner on programs from the late ’70s through the late ’90s, and he shared his memories with Variety of three particularly memorable TV performances. The first of these was in 1985, when she sang “What’s Love Got to Do With It” on the Grammys, the same year she won record of the year for that song — arguably the peak of her career, though she’d been revered for a quarter-century before that and would become a bigger superstar still in the next 25 years to follow. The second appearance he singles out is a duet between Turner and Elton John for a 1999 “VH1 Divas” special that got more fiery behind the scenes than necessary, albeit possibly to its benefit. Finally, he was an integral part of one of Turner’s last great TV spots, a highly copacetic duet with Beyoncé on the 2008 Grammys telecast.

Charna Flam Veteran stage and screen actor Gerald Castillo, who appeared in major TV series including “Saved By the Bell,” “General Hospital,” “Hill Street Blues,” “M*A*S*H” and “Dallas,” died May 4 at his home in Houston. He was 90. Known for his work as Major Slater on “Saved by the Bell” and Judge Davis Wagner on “General Hospital,” Castillo developed a following for his roles in the two series. Born in Chicago on Dec. 23, 1932, Gerald studied acting and stage direction at the Goodman Theater. Following his education, he acted on stages all across the nation, performing opposite Sherman Hemsley, Rita Moreno, Jessica Tandy, James Broderick and Jeanne Crain. After performing with Hemsley, “The Jeffersons” star convinced Castillo to pursue a film and TV career in Los Angeles.
