Fire service bosses were slammed at the Manchester Arena bombing inquiry after it emerged no commander was nominated until 90 minutes after the blast.

They were forced to deny accusations they tried to avoid responsibility, were 'neutered' by brigade policy and were accused by the chairman of the inquiry, Sir John Saunders, of 'taking no risks at all' in the effort to save lives.
